To secure a place on the course of your choice, please complete this form and return to Broadreach Training Services.  Payment should be received before the course can be confirmed for organisations.

Individuals funding their own training should include a deposit of 50%.  (Cheques made payable to Broadreach Business Ltd)

We reserve the right to refuse admission to any course.  We regret that no refunds are allowed once training confirmed/started.

If there are insufficient number for a course, it will be rescheduled to another date.  Non-attendance is still invoiced at full cost.

Those doing Health Care Course will need a DBS Disclosure paid for which is separate from the course fees.
